IMPORTANT INFORMATION ABOUT YOUR TICKET AND TRAVEL:
* Please review your documents and trip details thoroughly. If a discrepancy exists call Certified Travel/Destination Paradise Inc. immediately. Certified Travel/Destination Paradise, Inc. assumes no liability for any discrepancies not brought to our attention within 72 hours of invoice date.
* Please check in early. For domestic flights passengers should check in 90 minutes before scheduled departure time. Reservations for passengers checking in less than 30 minutes before scheduled departure are subject to cancellation. International departures require check in two (2) hours prior to departure; reservations are subject to cancellation 30 minutes prior to the scheduled departure time. Added airport security measures may require earlier check in and that you have a valid picture I.D. Ask your travel consultant for specifics.
* Denied boarding. Airline flights may be overbooked and there is a chance a seat will not be available on a confirmed flight. With few exceptions, persons denied boarding involuntarily are entitled to compensation by the airlines. The complete rules for payment of compensation are available at all airport ticket counters and boarding locations.
* Reconfirming continuing or return reservations. Should you miss a flight without having cancelled in advance, please reconfirm ongoing reservations to prevent automatic cancellation by the airlines.
* International flights. It is advisable to reconfirm your continuing or return reservations by notifying the airline at the city where you will board.
* Important notice about international travel with children. Minor children (age 18 and under) traveling from the United States to any foreign destination, including Bermuda, The Bahamas, and any Caribbean island, and Mexico, when not accompanied on the trip by both parents, must have a notarized affidavit from the parent not accompanying the child that:
1. The child is traveling out of the United States with the permission of the parent.
2. That the non-traveling parent is aware that the child is leaving on the departure date; and
3. The name(s) of the person(s) accompanying the child.
If the minor child is leaving the country without either of his/her parents, both parents must provide a notarization signed affidavit as outlined above. This affidavit cannot be signed more than 30 days prior to beginning travel.
* TRAVEL AGENT DISCLOSURE NOTICE: your travel reservations shall constitute consent to all of the provisions of this Disclosure Notice, and passengers are advised to take note of them.
In offering this air transportation or any other travel arrangements for sale, Destination Paradise, Inc. d/b/a Certified Travel (herein "Travel Agency") acts only as an agent. Travel Agency maintains no control over the personnel, equipment or operations of any supplier and assumes no resonsibility for, and shall not be expected, asked or required to pay for an financial loss, personal injury, property damage, other loss, accident, delay, inconvenience, or irregularity which you may experience either by reason of (1) nonperformance by any supplier for whatever reason; (2) wrongful, careless, negligent or unauthorized acts or omissions on the part of any supplier or its employees or on the part of any other party not under the control of Travel Agency; or (3) any defect, breakdown or other failure of any aircraft, train, automobile, ship, accommodation, or other equipment or instrumentality owned, operated or otherwise used by a supplier. Traveler assumes complete and full responsibility for, and hereby releases the agent from any duty of checking and verifying any and all passport, visa, vaccination, or other entry requirements of each destination, and all safety and security conditions of such destinations,during the length of the proposed travel. However, we specifically recommend that U.S. Citizens traveling to Canada, Mexico or the Caribbean, do so with a valid U.S. Passport. For information concerning possible dangers at international destinations, contact the Travel Advisory Section of the U.S. State Department, (202) 647-5225, or access the State Department's on-line travel advisory service (www.state.gov/travelinformation/travelwarnings). For medical information, call the U.S. Centers for Disease Control (CDC): (404) 332-4559 or log onto www.cdc.gov/travel. By embarking upon his/her travel, the traveler voluntarily assumes all risks involved in such travel, whether expected or unexpected. Traveler is hereby warned of the above risks as well as possible travel industry bankruptcies and medical and climatic disruptions, and the possibility traveler may be unable to travel as scheduled because of personal emergency. Traveler is advised to obtain appropriate insurance coverage against these risks; information is available through this agency regarding travel insurance. Traveler's retention of tickets, reservations, or bookings after issuance shall constitute a conset to the above and an agreement on his/her part to convey the contents hereto to his/her travel companions or group members.
Availability of refunds (if any) for unused travel reservations will vary with the type of reservations and the point at which reservations were not used. Please note that when you purchase travel services, your money immediately belongs to the supplier. In the event of any bankruptcy, you should seek refunds directly from the supplier or bankruptcy court. Trip cancellation and trip interruption insurance is available at an additional cost to protect you from certain unexpected events. The terms of insurance coverage are controlled by the actual insurance policy issued. Please ask your travel consultant for information regarding such coverage.
